20:51:01:03. Application requirements. An applicant for registration by examination shall provide the following to the board with the application:
(1) The application fee of thirty-five dollars;
(2) A photo of the applicant;
(3) A list of the applicant's practical experience;
(4) A transcript showing graduation from a college of pharmacy approved by the American Council on Pharmaceutical Education;
(5) A government-issued form of photo identification; and
(6) A criminal background check.
