\r\n\r\n

12:56:07:01. Required\r\napplicator records. Each applicator shall keep records which must include the\r\nfollowing for each application:

\r\n\r\n

(1) The name and\r\naddress of the person for whom the pesticide was applied;

\r\n\r\n

(2) The location of\r\nthe property where the pesticide was applied;

\r\n\r\n

(3) The pest to be\r\ntreated;

\r\n\r\n

(4) The acreage,\r\narea, or number of plants or animals treated or other appropriate description;

\r\n\r\n

(5) The year, month,\r\nday, and time the pesticide was applied;

\r\n\r\n

(6) The person who\r\napplied the pesticide;

\r\n\r\n

(7) The trade or\r\nbrand name and common name of the pesticide applied;

\r\n\r\n

(8) The company name\r\nappearing on the product label;

\r\n\r\n

(9) The weather\r\nconditions at the time of application, including the direction and estimated velocity\r\nof the wind and the temperature at the time the pesticide was applied except that this requirement\r\ndoes not apply to\r\nthe\r\napplication of bait in bait stations or pesticide applications in or\r\nimmediately adjacent to structures;

\r\n\r\n

(10) The amount of the\r\npesticide applied and\r\nits\r\nconcentration in pounds or gallons per unit or percentages of active ingredient\r\nper unit;

\r\n\r\n

(11) The specific crop or\r\ndesignated site or commodity to which the pesticide application was made.
