font-family:"Times New Roman",serif'>s</span><span style='font-size:12.0pt; font-family:"Times New Roman",serif'> one of the following conditions:</span></p> <p class=MsoNormal style='text-align:justify'><span style='font-size:12.0pt; font-family:"Times New Roman",serif'>&nbsp;</span></p> <p class=MsoNormal style='text-align:justify'><span style='font-size:12.0pt; font-family:"Times New Roman",serif'>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; (1)&nbsp;&nbsp;For a household whose heat is included in the rent or who pays rent plus heat, the household must provide the department with a copy of the eviction notice from the landlord for nonpayment</span><span style='font-size:12.0pt;font-family:"Times New Roman",serif'> of rent or heating costs</span><span style='font-size:12.0pt;font-family:"Times New Roman",serif'>;</span></p> <p class=MsoNormal style='text-align:justify'><span style='font-size:12.0pt; font-family:"Times New Roman",serif'>&nbsp;</span></p> <p class=MsoNormal style='text-align:justify'><span style='font-size:12.0pt; font-family:"Times New Roman",serif'>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; (2)&nbsp;&nbsp;For a household whose primary source of heat is natural gas or electricity, the household must provide the department with a copy of its disconnect notice from the supplier;</span><span style='font-size:12.0pt;font-family:"Times New Roman",serif'> or</span></p> <p class=MsoNormal style='text-align:justify'><span style='font-size:12.0pt; font-family:"Times New Roman",serif'>&nbsp;</span></p> <p class=MsoNormal style='text-align:justify'><span style='font-size:12.0pt; font-family:"Times New Roman",serif'>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; (3)&nbsp;&nbsp;For a household whose primary source of heat is fuel oil or propane, its fuel tank must be at least 80 percent empty and the fuel supplier must be requiring that a cash payment be made at the time the fuel is delivered.</span></p> <p class=MsoNormal style='text-align:justify'><span style='font-size:12.0pt; font-family:"Times New Roman",serif'>&nbsp;</span></p> <p class=MsoNormal style='text-align:justify'><span style='font-size:12.0pt; font-family:"Times New Roman",serif'>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; The department must approve expenditures made under the provisions of this chapter before the household incurs the expenses.</span></p> <p class=MsoNormal style='text-align:justify'><span style='font-size:12.0pt;
