Specific reasons for discontinuing</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0"> or denying</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0"> services include the following:</span></p><p dir="ltr" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1P0"><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1000002"> </span></p><p dir="ltr" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1P0"><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">    (1)  The individual does not meet eligibility requirements;</span></p><p dir="ltr" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1P0"><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">    (2)  The individual failed to cooperate with the needs assessment;</span></p><p dir="ltr" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1P0"><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">    (3)  The </span><span class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">individual</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0"> is sexually harassing, verbally abusive, threatening, or combative;</span></p><p dir="ltr" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1P0"><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">    (4)  The </span><span class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">individual</span><span class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">'s</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0"> personal</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0"> care </span><span class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">service needs</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0"> exceed the</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0"> service</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0"> limits of the </span><span class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">program</span><span class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">;</span></p><p dir="ltr" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1P0"><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">    (5)  The </span><span class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">individual</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">'s </span><span class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">physical</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0"> environment presents health and fire hazards or unsafe conditions;</span></p><p dir="ltr" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1P0"><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">    (6)  The </span><span class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">individual</span><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0"> is not in compliance with the case service plan;</span></p><p dir="ltr" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1P0"><span xml:space="preserve" class="s0db3a5a9-5bf5-4216-9226-b6f4a99c7bb1C0">    (7)  The individual's health and safety risk factors are unable to be mitigated;
