The case manager must assess the individual's cognitive, mental, and physical functioning capabilities within 45 days to determine whether the individual is appropriate for assistive daily living services.</font></p> <p><font size=3 face="Times New Roman">&nbsp;</font></p> <p><font size=3 face="Times New Roman">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; If determined appropriate, the case manager together with the client shall prepare the assessment and service plan. At a minimum, the service plan must contain the types of services to be furnished; the amount, frequency, and duration of each service; and the type of provider to furnish each service. The case service plan must be reviewed and approved by the client's personal physician. As evidence of approval, the client's physician must sign and date the client assessment and service plan. The case manager must forward copies of the completed client assessment and service plan to the Department of Human Services.</font></p> <p><font size=3 face="Times New Roman">&nbsp;</font></p> <p><font size=3 face="Times New Roman">&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; The case manager is responsible for ongoing monitoring of the services included in the client's assessment and service plan. Additionally, case managers must initiate and oversee the process of assessment and reassessment of the client's level of care.
