To obtain licensure for a crematory, a person must:</span></p><p dir="ltr" class="sf8e3b20d506c4a2bb5fc0682d4a218f9StatuteNumber1"><span class="sf8e3b20d506c4a2bb5fc0682d4a218f9DefaultParagraphFont">(1)    Submit an application to the board, at the time and in the manner required by the board;</span></p><p dir="ltr" class="sf8e3b20d506c4a2bb5fc0682d4a218f9StatuteNumber1"><span class="sf8e3b20d506c4a2bb5fc0682d4a218f9DefaultParagraphFont">(2)    Identify, on the application, all certified crematory operators employed at the crematory;</span></p><p dir="ltr" class="sf8e3b20d506c4a2bb5fc0682d4a218f9StatuteNumber1"><span class="sf8e3b20d506c4a2bb5fc0682d4a218f9DefaultParagraphFont">(3)    Provide evidence that a motorized or mechanical device is available for the processing of cremated remains;
