FOR AN ACT ENTITLED, An Act to
require the acceptance of certain uniform life insurance
request forms.
BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF SOUTH DAKOTA:
Section 1. That chapter 58-15 be amended by adding thereto a NEW SECTION to read as follows:

Section 1. That chapter 58-15 be amended by adding thereto a NEW SECTION to read as follows:
The director shall develop uniform life insurance forms for requests to cash surrender, to
obtain maximum policy loans, and for Section 1035 exchanges. A life insurer shall accept the
uniform life insurance forms and may not require the use of forms in addition to the uniform life
insurance forms.
