47:04:01:01. Form required for claims. All claims for unpaid wages under SDCL 60-11 shall be filed with the division in writing. A simple letter stating the facts, such as amount owed, name and address of the employer, and period of time worked is sufficient.

47:04:01:06. Assignment of wage claim. The division after having determined that a claim for wages is valid may take assignment of the wage claim on forms approved by the division.
